The Department of Health and Human Services (DHHS) has appointed Nexus Primary Health (Nexus) as the new provider to administer the Home and Community Care Program for Younger People (HACC-PYP) in the Shepparton area from 1 December 2018.
The HACC-PYP is a Victorian Government program which has been administered by Council for many years. With the National Disability Insurance Scheme (NDIS) roll out in Shepparton from January 2019, Council made a decision to withdraw from providing HACC-PYP services as a significant proportion of clients are expected to transition to the NDIS, resulting in a reduced program that is no longer viable for Council to deliver.
Greater Shepparton City Council Manager Neighbourhoods, Amanda Tingay, said “Nexus brings considerable experience to our area as they have been providing these services in Strathbogie and Mitchell Shires for many years.”
Calvary Community Care delivers the in-home services on behalf of Greater Shepparton City Council which includes the HACC-PYP. “When Nexus commences on 1 December 2018, Calvary Community Care will no longer deliver services through the HACC-PYP,” said Ms Tingay. “This potentially will mean a change of support worker for clients; however Nexus will discuss this directly with each client should they be affected by the change.”
“We understand the change may be difficult for clients and their families,” she said. Greater Shepparton City Council will be working with Calvary Community Care, the Department of Health and Human Services, and Nexus to ensure clients receive continuity of care with minimal disruption.”
